Providing quality confidential home nursing, primary health initiatives and referral to other community services. The service can be accessed by self referral.
Some of the services are provided by Murray Mallee Community Health Services.

Services Provided by the Outreach Team:Home Nursing
(e.g. showers, dressings, nursing care and carer support)
In Home Respite and Centre Based Respite
In Tailem Bend and outlying areas.
Meals on Wheels
To provide the community with help with the provision of quality home delivered meals. Mon-Fri with the exception of public holidays. Outreach referral required.
Telecross
To provide a brief friendly telephone call - sharing security and reassurance service for frail, aged, disabled and socially isolated.

Domicillary Care
To provide a quality home cleaning service for disabled, frail or aged. Outreach referral required.
Home Linen Supply
Bed linen may be provided by Outreach for special needs.
Community Education
Primary Health and awareness campaigns.
